Terms and Conditions for Trunk Show

                                 Event Date: Saturday, June 7, 2025 | 9:00 AM – 2:00 PM

By participating in the North Ave Eye Care Trunk Show Event, you agree to the following terms and conditions:

1. Event Credit Usage

• RSVP and referral credits are valid only on Saturday, June 7, 2025, during event hours (9 AM – 2 PM).
• Credits may be used only toward prescription eyeglasses purchases (frame + lenses).
• Credits have no cash value and are non-transferable.
• Credits cannot be saved, transferred, or used on another day—no exceptions.

2. Credit & Discount Limits

• A maximum of $200 in credit may be applied per eyeglass purchase.
• This includes a combination of RSVP and referral credits.
• Insurance benefits and credits may not be combined.
  • You may choose to use either your insurance or the event credit, but not both on the same glasses pair.

  3. Referral Credit Terms

• A $30 referral credit is applied when a new patient:
  • Mentions your name at the time of their appointment or
  • Texts us their name with your referral prior to the event.
• Referrals must complete an eye exam for the credit to be valid.
• Referrals must be new patients only (not existing patients).
• Multiple referrals are allowed.

4. Bonus Lens Discount

• Clients who makes a purchase  even with insurance will receive 50% off all extra lenses (e.g., sunglasses lenses, backup glasses, blue light filters, etc.) if purchased within 90 days of the original purchase date.

5. Product Availability

• Eyewear collections from PRADA, COACH, and TOM FORD will be available in-store for viewing and purchase only during event hours.

6. General Conditions

• Offers valid in-store only on June 7, 2025.
• Offers are subject to change or cancellation without notice.
• No rain checks, extensions, or substitutions.
• Credits and discounts are applicable only to the person named in the RSVP or referral.

7. Appointment & Exam Requirements

• An eye exam is required to redeem credits toward prescription glasses unless a valid prescription is already on file or presented at time of purchase.
